How to Enroll

1) Check Your Qualifications

To enroll in the Valir PACE program, you must meet the following criteria:

Enrollment in PACE is voluntary. To enroll, you or your loved one must meet specific eligibility criteria. To qualify, you must:

  • Be 55 or older
  • Live in a PACE service area
    • Oklahoma City Adult Day Center
    • Shawnee Adult Day Center
  • Be certified by your state to need nursing home-level care
  • Be able to continue living in the community safely with the help of PACE services

A physical and medical assessment is done by the PACE team and approved by an OKHCA staff member. No financial fees or costs are associated with determining/applying for Valir PACE. If you qualify for Medicare or Medicaid, these agencies will help pay for the cost of PACE services.

Valir PACE participants must receive all care, including primary and specialist other than emergency services from PACE. Participants may be fully liable for costs of unauthorized or out-of-PACE program services. No deductible/co-payment is required for services approved by the Valir PACE interdisciplinary team.

What to Expect When Enrolling in PACE

If you meet the qualifications and decide to enroll, know that you will be asked to provide medical and other personal information needed for your care team to assess your needs and services needed.  

Once you sign an enrollment agreement for PACE, you will receive more detailed information on your personalized care plan, how to get services, and plans for emergency care if ever needed.

You do not need to reenroll annually as long as you continue to meet the program’s criteria.  You can disenroll from the PACE program at any time, regardless of Medicare enrollment periods. Once enrolled in PACE, there are no co-pays, deductibles, and no gap payments.
