What is PACE, and What Services Does it Provide?

PACE is a comprehensive medical and social services program to help older adults with long-term care needs live outside a nursing home or similar facility. It provides a host of low-cost services addressing various needs, from everyday healthcare to social opportunities. Here are just some of the services offered through the PACE center at Valir Health:

  • Primary and hospital care
  • Medical specialty services
  • Hospice care
  • Home health care
  • Counseling and other social services
  • Hot meals
  • Medical transportation
  • Access to a wellness center
  • Field trips out in the community
  • And much more!

PACE is available to anyone 55 years older who requires nursing home levels of care (although you do not need to live in a nursing home), demonstrates financial need as determined by the state, and lives in the service area.

PACE’s holistic approach to healthcare means that it offers several unexpected benefits. While medical care is a large part of the program, it’s by no means the only part, and PACE can play a significant role in helping you age healthfully.

Fun With Friends:

Did you know that social isolation among older adults can increase the risk of heart disease, depression, and cognitive decline? PACE moves beyond simple medical care to encompass opportunities for socialization, both at our senior wellness center and through fun field trips across Oklahoma.

Our wellness center offers card games, board games, arts and crafts, movies, and more. We also have a gym where you can work out with friends. Most importantly, we send out weekly calendars so you always know what’s coming up and can make sure you join us!

Good Eating:

As you age, preparing healthy, nutritious meals can become challenging. PACE offers several services to ensure you can meet all your nutritional needs. We provide hot meals and snacks at our wellness center, so you don’t have to worry about going without.

PACE also includes access to a nutritionist so you can learn more about which dietary choices are best for your symptoms and conditions. We can also provide tips and strategies for preparing healthy meals at home.

For those who might be at risk for food insecurity, we can work with you to ensure you’re receiving any necessary assistance, such as access to a speech therapist to help with chewing and swallowing or transportation to and from the grocery store.

Continue Living at Home:

One of the primary goals of PACE is to help older adults continue to live in their homes rather than moving into a nursing home or other care facility. PACE offers the same benefits you can find at a nursing home (24-hour care access, social events, transportation) through our wellness center–but without requiring you to give up your own space.

We also offer home care services to help with anything that needs to be done at your home. Our home health team can assist with household responsibilities and offers suggestions for modifications to your home, such as guard rails, to help you maximize your mobility.

Comprehensive Health Care

Regular medical check-ups are a crucial part of aging healthfully for older adults, especially those with chronic health conditions. PACE not only provides access to primary care services but will ensure you have transportation to and from your appointments as well!

PACE doesn’t just cover day-to-day healthcare needs, though. Through the program, you’ll have access to the following at limited cost:
